Data Request Form

The Office of Institutional Effectiveness (OIE) frequently receives requests for different types of reports containing college related data. Our goal is to provide faculty, staff and students with accurate information needed for the success of each project.

It is both the OIE, as well as the requesting office's responsibility to maintain the integrity and security of the information. If the data requested will be used as part of a research project, the individual/s requesting the report should obtain prior approval from the Institutional Review Board (IRB). Provided the IRB has approved your research, the OIE will process the request internally, or by contacting different departments across campus such as the Registrar's Office or the IT Department. Please keep in mind that data reports are time consuming and often require complex processes from various departments across campus, therefore your requests should be submitted with a considerable amount of time.

In order to obtain consistent, accurate, reliable and reportable data it is imperative that you request updated reports from the OIE, rather than updating the data yourself.

Never release data to the public, quote data or publish data with the express consent of the Office of Instutional Effectiveness.
